Die Borderline-Persönlichkeitsstörung (BPS) zeichnet sich durch ein komplexes, tiefgreifendes Muster von Instabilität in Selbstbild, Verhalten und Beziehungen der Betroffenen aus. Die Störung stellt eine massive Herausforderung für die Betroffenen, Angehörigen sowie Behandelnden dar. Vor allem für schwer kranke Patientinnen und Patienten fehlen ambulante Behandlungsmöglichkeiten, die der Komplexität der Störung gerecht werden und hinreichend "krisenfest" sind. Obwohl die Behandlungsleitlinien explizit eine Komplexbehandlung im Sinne einer Integrierten Versorgung empfehlen, fehlten derartige Angebote bislang, so dass immer noch der größte Teil der in Deutschland für die BPS aufgewendeten Mittel in stationäre Kriseninterventionen fließt. Die Integrierte Versorgung (IV) – Borderline setzt nun die Leitlinienempfehlung in ein auf der Dialektisch-Behavioralen Therapie (DBT)-basierendes, multimodales, langfristig angelegtes und störungsspezifisches Behandlungskonzept um. Vordringliche Ziele sind die Förderung einer funktionalen Selbstregulation, die Verbesserung der Lebensqualität, der Abbau selbstschädigenden Verhaltens und die Reduktion stationärer Kriseninterventionen. Zentrales Element ist ein ambulantes, interdisziplinäres IV-Kernteam, das in einem psychiatrischen Krankenhaus mit regionalem Versorgungsauftrag arbeitet und dessen Strukturen nutzt. Inhaltlich arbeitet dieses Team als DBT-Team. Es bietet DBT-Einzel- und Gruppentherapie, Krisenintervention, Psychopharmakotherapie und sozialpädagogische Unterstützung aus einer Hand. Ersten Evaluationsdaten zufolge scheint das Konzept in der Lage zu sein, schwer kranke BPS-Patientinnen und -Patienten weitestgehend ambulant erfolgreich zu behandeln. Die S3-Leitlinie „Posttraumatische Belastungsstörung“ (PTBS) wurde in ihrer aktualisierten Version von 2019 von zahlreichen Fachgesellschaften unter der Federführung der Deutschsprachigen Gesellschaft für Psychotraumatologie (DeGPT) erarbeitet. Sie enthält gegenüber der Vorversion verschiedene Neuerungen: Aufgrund der Einführung der Diagnose „Komplexe PTBS“ in ICD-11 widmet sich ein neues Kapitel der Behandlung dieser Störung. Auch den neuen Befunden zur Behandlung der PTBS bei Betroffenen mit anderen psychischen Diagnosen wird in einem eigenen Kapitel Rechnung getragen. Darüber hinaus wurde ein eigener Teil der Leitlinie zur „Diagnostik und Behandlung der Posttraumatischen Belastungsstörung bei Kindern- und Jugendlichen“ aufgenommen, der auf spezifische Fragestellungen bei der Behandlung von Kindern und Jugendlichen eingeht. Die S3-Leitlinie „Posttraumatische Belastungsstörung“ ist damit noch umfassender geworden und gibt allen Berufsgruppen Empfehlungen an die Hand, die in die Betreuung von Patienten mit Posttraumatischen Belastungsstörungen eingebunden sind.
