Cancer is a global burden with more than 18 million diagnosed and an estimate of 10 million deaths in 2020 worldwide. Cancer continues to be a major and growing problem in conflict affected areas too. The incidence of cancer is expected to rise significantly in those countries compared to the rest of the world. Conflict, massive migration and displacement has put a tremendous pressure on all health care systems and health economy which halted improvement in many cancer care in majority of the countries. Countries affected by conflict like Syria, Ukraine, Iraq, Yemen, Latin America and others has led to destruction of the whole countries healthcare infrastructure including health care facilities, diagnostic facilities, and unavailability of drugs and loss of health care professionals due to death, migration or disabilities. This lead to inequitable access to care, lack of prevention, loss of screening programs, delay in diagnosis, loss of follow up and in many cases no anticancer therapy or palliative care to offer. Historically, most of humanitarian aid was focused on basic needs and first aid, leaving NCDs including cancer care with trivial support if any. Recently with the Russian invasion of Ukraine and the negative global economic crisis, refugees all over the globe will have devastating impact on cancer care screening, early detection, treatment and palliative care.

Volume One of Richard Sullivan's Trilogy is a sweeping historical novel of the Irish-American grab for power in Buffalo NY in the 19th Century and the personalities involved: newspaper editors, politicians, thugs and innocents. Newlywed Sam Clemens, a.k.a. Mark Twain, was gifted with a spledid home with servants, as well as a part ownership of the Buffalo Express newspaper by his generous father-in-law, yet the Great American Storyteller would find neither happiness nor success in this unruly city. When First Ward dock-walloper Fingy Conners' family members all died mysteriously within a single year, he inherited everything, including his father's saloon. Using his saloon as the key, he set out to control labor contracting on Buffalo's docks. So overwhelming was his iron-handed saloon-boss system that within a few years he controlled the entirety of shipping on the Great Lakes, ascending to enormous wealth and power in less than a decade, defrauding voters, installing his own puppet politicians, and dominating the entire Buffalo Police Department. By hiring, paying, feeding, watering and boarding laborers out of his saloons, Conners enslaved thousands of families in Buffalo and all around the Great Lakes in misery and hunger for two decades. After the Sullivan Brothers were placed in an orphanage by their destitute mother following the death of their Union soldier father in the Civil War, poverty, insecurity and violence infected their lives. John P. Sullivan, the city's powerful First Ward alderman, was installed in that office by Fingy Conners and held it for a quarter century. The brothers grew up with Conners and maintained their troubled alliance with the saloon-boss throughout their lives. Brother James' fortuitous encounter with Mark Twain as a boy, soon after the famous author moved to Buffalo to edit the Buffalo Express newspaper, and the friendship it initiated, would have a remarkable in uence on James for the rest of his life. As Detective Sergeant James E. Sullivan of the Buffalo Police Department, Jim lacked his brother's blind ambition, and found himself caught up amid forces he could not surmount. He was compelled to follow the orders of his Sheehan-Conners controlled superiors and to rescue his brother from the endless messes the Alderman created for himself. Jack White, secret murderer and Boston politician, was Buffalo's most powerful alderman, ever. Posing as a Republican, White helped pave the way for the rise of Democrats Sheehan, Sullivan and Conners. But once he'd served his purpose, his former allies swiftly did away with him. In the middle of this maelstrom are the Sullivan wives; the Alderman's Annie, who is blinded to what's transpiring around her by the perks she enjoys due to her husband's status, and the Detective's Hannah, who is rewarded with little more an endless stream of grief and frustration for standing by her spouse.

The First Ward

Volume 5 of Richard Sullivan's The First Ward historical novel series, "Everybody Dies In The End," based on actual people and their activities legal and otherwise in Buffalo's wild and wooly waterfront First Ward, sees five of the real life principal characters die-four of them within four months of each other. Preceding their demise, each is involved in a series of life-changing ventures. We meet William J. "Wild Bill" Donovan, known as the "Father of the CIA" who was born and raised in the First Ward. Donovan lets others' high opinions of him go to his head as he overreaches his authority to betray and alienate his family, friends, clients and even his own law partner. Future King of England, Edward Prince of Wales, visits Buffalo with his flirtatious gay brother to dedicate the opening of the Peace Bridge linking Canada and the US. Fingy Conners, "The Boss" of the First Ward, takes control of the Great Lakes shipping trade, putting him in the ideal position to make millions rum-running during Prohibition. During Prohibition the US Coast Guard runs wild murdering innocent citizens merely on suspicion of their possessing alcohol. Lydia Harper, a beautiful negro prostitute, becomes a heroine in the deadly cholera epidemic, only to be shunned afterward by the very same people whose lives she saved. In his final year on earth, Detective Sergeant Jim Sullivan resurrects the Mutual Rowing Club after World War I, coupled with the Spanish Flu epidemic, nearly cause its closure. The widow of a murdered sailor enlists Alderman John P. Sullivan in her quarter century battle to clear her husband's name and win her rightful widow's pension from the US Navy. During a labor strike against him, Fingy Conners imports 12,000 desperate men into the city as strike-breakers, who then go on to wreak havoc at great cost to city taxpayers. Members of the Mutual Rowing Club sabotage their own National Champion's chances to compete in the 1924 Paris Olympics. A Buffalo pharmacist, Boris Reinstein, joins the Russian Revolution and rises to become a principal Minister in Vladimir Lenin's Soviet cabinet. Publisher William Randolph Hearst once again decides to run for Governor of New York and enlists Fingy Conners as his campaign manager. Movie star Marion Davies, Hearst's out-of-control mistress, poses the ultimate threat, as the Catholic Bishop of Buffalo William Turner just happens to occupy the ship's cabin next door to Hearst and Davies' during their clandestine (and very noisy) trans-Atlantic crossing. And through it all, the conscience of The First Ward series, Hannah Sullivan, endures and ultimately triumphs in her own modest manner.
