Revalidation Portfolio

A log book to record practice hours and continuing professional development (CPD) for the new Nursing and Midwifery Council (NMC) requirements starting in April 2016. Nurses will ne longer be able to keep their registration if they cannot prove they have fulfilled all requirements. This log book will help keep track of all of your learning and practice hours according to the NMC requirements. Use this book to present to your confirmer that you have met the requirements. Also included are pages for planning for your reflective accounts and a notes section in the back.
